Membership Policy for Patients with Commercial Insurance

As of January 1, 2022

 

You are part of a unique Lifestyle Medicine practice that engages, educates, and empowers you to be healthy. We do this through longer appointments, deeper relationships with providers and staff, and easy accessibility.  In order to keep you well, and to also be financially sustainable, we are asking you to partner with us as follows:

 

  • Annual Wellness Visits (in person) are intended to prevent illness and detect health concerns early, before symptoms are noticeable and when they can be treated more easily.  We expect patients to have wellness exams at our clinic every calendar year in order to remain a member at the clinic.  Most insurance plans cover this.  Please note:

    • Labs will be ordered during the Annual Wellness Visit or in a previous follow-up appointment to be sure we are ordering all relevant blood work.  

    • Most commercial insurance plans don’t cover care for acute problems during your wellness visit, so please schedule sick visits on a separate day if you’d like it to be covered.

    • For women who have their well-woman exams with their gynecologist, insurance allows women to also have a medical wellness exam with their primary care provider.

 

  • Lab/Imaging Result Visits (in person or video) will be scheduled separately to allow providers the time to explain your test results thoroughly, answer your questions, and make a comprehensive treatment plan that is tailored to you and your needs while addressing abnormal or suboptimal results. All testing orders will require a follow up appointment for discussing results, unless results are completely normal.  

 

  • Sick Visits and Follow-up Visits (in person or video) are to discuss new or existing health issues, concerns, or symptoms. Your provider may order tests, prescribe medication, refer you to a specialist, or provide advice and education. Office visits are generally covered with a standard insurance copay, coinsurance or deductible, depending on your plan. 

 

  • New Prescriptions and Some Refills (in person or video):

    • All NEW prescriptions require a visit.

    • All controlled substance prescriptions require minimum quarterly follow up visits for refills. 

    • All prescriptions for chronic conditions require minimum semiannual follow-up visits, or more, if needed.   

 

  • FMLA forms or other illness-related paperwork (in person or video) are time-consuming and require a visit where we co-complete the form and cover the time it takes for our staff to submit it.
     

  • New referrals and therapy orders require a visit (in person or video) to evaluate and document the issue and to cover the time required to submit the order.

There are no additional fees for members.   We are only asking for your participation.
